**Employment Type:** Part time **Shift:** **Description:** Hospice Nurse Practitioner - Contingent Saint Agnes Home Health and Hospice | _Part of Trinity Health At Home_ Make a difference where it matters most-at home. We're looking for a compassionate Nurse Practitioner to provide expert hospice care in both home and hospital settings. You'll collaborate with an interdisciplinary team to assess patient eligibility, manage symptoms, and support families with dignity and grace. Key Responsibilities + Conduct face-to-face patient assessments + Collaborate with physicians and care teams on hospice plans + Prescribe medications and order diagnostics + Mentor staff and guide care decisions + Ensure compliance with hospice regulations and ethical standards ✨ Position Highlights & Benefits + 403(b) with matching contribution + Comprehensive orientation and onboarding + Pay range: $54-$85/hour What You Bring + Master's in Nursing & NP certification + 5+ years of nursing experience (hospice, oncology, geriatrics preferred) + DEA license & active medical staff membership + Strong communication and emotional support skills + Valid driver's license and reliable transportation Why Join Us?
